Business notes: The #2 US supermarket chain (behind Kroger), Albertson's operates more than 2,500 stores in 37 states under names such as Albertson's, Acme Markets, Jewel, Shaw's and Star Markets. (Albertson's acquired Shaw's from J Sainsbury in 2004 and announced it will be closing its New Orleans stores by the end of 2004) More than half are combination food stores and drugstores (most with floral, bakery, and video departments), and about 225 have gas stations. Also please include the following information with your grant proposal:
1. Board of Directors (names, occupations, & community affiliations of board members as well as committee assignments & criteria for board selection)
2. Finances (current annual operation budget, with summary of itemized revenues & expenses)
3. Funding sources for the organization, with listing of past major contributions, including amounts, & anticipated future funding sources
4. Annual Report, (most recent annual financial statement)
5. Copy of original IRS determination letter indicating tax-exempt status

2. Description of program goals and measurable objectives
3. Description of program and activities planned to accomplish these goals; description of whether this is a new or ongoing activity
4. Timetable for implementation
5. Other organizations, if any, participating in the program
6. List of names and qualifications of key staff and volunteers responsible for project implementation
7. Long-term sources and strategies for funding of this project
8. Projected project budget

C. Evaluation:
1. Expected results during the funding period
2. How success will be defined and how it will be measured
3. How the project's results will be used and disseminated

When an organization seeks to renew a contribution, information will be required on how Jewel-Osco's previous donations were used and the value they provided. Program effectiveness and results will be used in evaluating requests for future grants.
